This can be used as a digital product with text boxes ready to go or you can print the document if you prefer a paper copy.

This log aligns to the NESA requirements for logging elective professional development at the time this product was created. 

What is included: A page for your personal details; pages with different table options according to your preference of recording. this includes a mix of sections to insert length of the PD, date completed, the type of activity it was, the standard/s that PD addressed, as well as the option for a section to specify if it was a professional commitment activity (some levels of accreditation require this).
